Verifying Your Insurance and Filing Claims
Please be sure to bring your insurance cards and some form of personal identification with you to every visit. Also, be sure that you understand the type of health insurance policy you have and the coverage it provides. You should also take care of any requirements your carrier has regarding pre-certifications, authorizations or second opinions. Failure to meet these requirements may result in a claim denial or higher co-payment.

As a courtesy, we will handle the billing paperwork and bill your insurance carrier for you. You will receive periodic statements indicating the status of your account. Ultimately, you are responsible for any charges not paid by your insurance carrier.

If you do not have insurance, payment in full is required within 15 days from receipt of the bill. If you cannot meet this schedule, one of our financial counselors will be happy to help arrange a convenient payment plan. We accept cash, checks, Visa, and MasterCard.

Medicare
Medical Surgical Specialists' physicians participate in the Medicare program. This means we will bill Medicare on your behalf. Medicare usually reimburses 80% of this amount, and the patient is responsible for the remaining 20%. Many patients carry supplemental coverage (or Medigap) that cover Medicare deductibles and co-insurance amounts. As a courtesy, we will also file your co-insurance claims.

Medicare may not pay for some preventive or screening services. We will identify these procedures and advise you in advance so you can make an informed decision. Please note that you will be responsible for any charges not covered by Medicare or your co-insurance carrier for these tests.

Commercial Insurance
We will work with you to confirm or verify your insurance benefits. If you belong to a plan that requires pre-authorization, you must provide an authorization number prior to each appointment. Please retain this authorization number for your records.

Deductibles or services not covered by your policy will be noted on your monthly statement. Payment may be made by cash, check, VISA, MASTERCARD, or AMERICAN EXPRESS.

Managed Care Plans
Medical Surgical Specialists participates in selected managed care plans. If you are insured through such a plan, please check your provider's handbook or call your provider for an updated list of Medical Surgical Specialists' physicians who participate.

Worker's Compensation
Before you can make an appointment with a Medical Surgical Specialists' physician, your employer or Worker's Compensation carrier must authorize your visit and notify our Worker's Compensation Coordinator. This authorization must state the date and type of accident you had, as well as who will assume responsibility for charges incurred in rendering your care. It should also state the name and address of where the bill for your services should be sent.

No Insurance or Limited Coverage
If you have no insurance, you will be required to make a deposit of $250 on the day of your visit. This may be done by cash, check, VISA, MASTERCARD or AMERICAN EXPRESS.

Insurance Filed Outside the United States
If your insurance is filed outside the United States, you will be expected to make a deposit equaling an estimate of your charges on the day of your appointment unless other arrangements are made in advance. We will be happy to supply the appropriate forms to complete your claim.
